Refer a patient

Thank you for referring your patient to us. Our scope of practice, imaging policy and how we report back are set out in our referral guidelines.

Four ways to refer

Text. Photograph your referral and text it to 0499 024 080. This is how most referrals reach us and it works well.

Email. admin@gvendo.com. Please note that email is not a secure channel. If you would prefer, text instead.

The form below. Fill it in and we will phone your Practice for the rest.

Referral pad. Call us on 0499 024 080 and we will post you a pad.

Sending radiographs

Please send images by text to 0499 024 080 or by email to admin@gvendo.com.

We have deliberately removed file uploads from the form below. A public web form that accepts clinical images creates a storage and data residency question, and text/email is preferable.

Please include the patient’s first name and date of birth with any image so we can match it to the referral.

Referral form

Why we ask for only part of the patient’s name. This is a public form on the internet, so we keep the identifying information on it to the minimum that still works. Our receptionist will phone your Practice for the patient’s full surname, address and contact number. That call also confirms the referral came from you.
